Section A: Applicant Acknowledgement & Declaration
1. In consideration of the grant of SSG and/or any government funding to me for the Course applied for herein, I hereby undertake as follows:
a. I will fulfil a minimum of 75% attendance for the Course as well as, where applicable, sit for and/or pass all required Course assessments (“Minimum Requirement”).
b. For self-sponsoring applicants: In the event that I fail to meet the Minimum Requirement, I will be liable for the total amount of the Course fee and I will forthwith pay SFIC Institute (SFICI), without demand, the amount equivalent to the SSG and/or any government funding.
c. For company-sponsored applicants: In the event that I fail to meet the Minimum Requirement and/or I cease to be employed by the company named in Section C during the Course, I will be liable for the total amount of the Course fee, and I will forthwith pay the SFICI, without demand.
d. If I fail to pay any amount due from me to SFICI within the stipulated deadline, SFICI shall have the right to impose a reasonable administrative charge and bank interest on the outstanding amount. SFICI shall have the right to suspend me from the Course and/or withhold the Course Certificate until I have paid all outstandings in full.
2. I hereby declare that:
a. I have not previously received any subsidy for this Course from SSG and/or any government agencies through any other training provider.
b. I have not received any non-government incentives or other privileges to take up this course.
c. The information provided by me herein is complete, true and accurate. Any misrepresentation or omission may lead to rejection of this application and/or disqualification for any funding and SFICI shall be entitled to terminate my enrolment in the Course without a refund of Course fees.
3. I agree that SFICI may collect, use and/or disclose my personal data provided in this form for matters relating to my Course enrolment, account servicing of course-related activities, reporting to funding or other relevant agencies/bodies/organisations, conduct of statistical research and surveys to administer, develop or improve the Course and for the conduct of SFICI business in accordance with the Personal Data Protection Act 2012.
4. I agree that I may be contacted via mobile phone and/or email and on occasion SFICI may text and/or email me information related to the Course or events at SFICI during or after the completion of the Course. My personal data may be kept by SFICI for a limited number of years after I have completed the Course in order to fulfill requests from WSG/ SSG.
Section B: Company Representative Acknowledgement & Declaration (For Company Sponsored or CCP Programme Only)
1. Training is fully sponsored by the company which is registered or incorporated in Singapore.
2. Payment must be received in full prior to programme/course commencement.
3. Sponsored employee must be a Singaporean Citizen or Singapore Permanent Resident and attend both training and final assessment (FA) on the assigned dates and achieve a minimum 75% attendance per training module.
4. SFIC Institute is not liable if applicant fails to meet at least 75% attendance per training module, does not complete assessments, or for any reason where Course Fee Support/Absentee Payroll Funding is not submitted or approved. In the scenario that the applicant is deemed ineligible for funding by Workforce Singapore (WSG) and/or SkillsFuture Singapore (SSG), the sponsoring company is liable to pay SFIC Institute the Funded Portion.
5. Absentee Payroll Funding is applicable to company-sponsored applications at prevailing SSG stipulated rates.
Section C: Important Information (Please read this before submitting the application form)
1. Ensure you are aware of and have met the pre-requisites of the courses/programmes you are signing up with SFIC Institute Pte Ltd.
2. Ensure that you have completed all fields in this application form before signing up with SFIC Institute Pte Ltd. Failure to do so will result in your registration being null and voided.
3. Enrolment and Confirmation
a. Confirmation of enrolment for any courses/programmes will only be made upon receipt of full payment, application form and complete supporting documents (subject to seat availability).
b. In event that the class is cancelled, a notification email will be sent to you at least 3 days before course commencement.
4. Withdrawal, Re-scheduling & Refund Policy
a. Applicant must adhere to the schedule of course/programme selected including final assessment as there will be no makeup session(s).
b. Refund of course fees will be based on the following conditions:

5. Rescheduling of Final Assessment (FA) will incur a charge of $130.80 and subject to availability of trainer, venue and dates. FA can only be rescheduled to a date within the training period. Fees will be forfeited should the Applicant fail to show up on the rescheduled date and he/she will be regarded as “NYC” (Not Yet Competent). The applicant will then be liable to pay full course fees (without subsidy). ^Working days: Monday to Friday (Excluding Sat, Sun & Public Holidays).
6. Appeal and Re-assessment
a. In the case of making appeals concerning FA results, the applicant must submit his/her request in writing and spell out the reasons for the appeal with relevant documents within 1 week after the last FA. A non-refundable handling fee of $54.50 will be charged.
b. In the case of a re-assessment, a FA fee of $130.80 is payable at the point of request.
7. SFIC Institute Pte Ltd reserves the right to amend the terms and conditions without prior notice or to cancel/re-schedule the class or change the course fee/venue as and when it is considered appropriate or the circumstances so require. SFIC Institute Pte Ltd’s decision shall be final.
